Phelps Medical mChart

Phelps Medical MyChart® is a patient interface that is easy to use, comprehensive, and secure. You can use a PC, tablet, or smartphone to access your medical records with MyChart. To register for or log into your MyChart, which patient portal account, click the link below.

To create an account on MyChart, even if you have previously used one of the Phelps Health client portals (Passports, healow, eCW, etc.), you need to sign up as a New User. Your MyChart account will receive some of the data from those additional patient portals, including allergies, prescriptions, and vaccination records.

Using MyChart, the following tasks are possible:

Examine the results

. Display evidence of COVID-19 immunisation status or test findings using MyChart.

Use MyChart to access patient education

During your clinic appointment, you will receive an activation code that you can use to access MyChart. You can use this code to sign in and set your own password and username.

If you were not given an activation code, ask to sign up on the next office visit or give your usual care clinic a call to obtain one.

You can securely communicate on behalf of a close family member or someone you are caring for with a patient’s provider’s office by using proxy access. In order to obtain proxy status to a relative’s MyChart consideration you must first complete one of the following forms:

Note that you can only request documents related to your own accounts at this time; records on gateway accounts (accounts you administer on behalf of an individual else) cannot be requested.

Your records can be mailed, faxed, or e-mailed to you, or they can be sent directly to your MyChart account.

Instantaneous Results

With the implementation of the immediate release of MyChart results starting on October 6, 2022, the majority of your medical records and test results will be accessible through the Phelps Health MyChart, which app as soon as they are completed. You get immediate access to the results on MyChart, allowing you to view them before your physician. It might be a good idea to hold off on messaging your provider until after a few days have passed.
